Schedule an Appointment

To schedule an appointment call 701.364.4040.  If we are in session and unable to answer your phone call, please leave a message.  We will return your call as soon as possible.  We’re sorry, but due to confidentiality, we do not schedule appointments over email.

Client Forms:

If you are a new client, you may complete the following forms and bring them to you first appointment, or we have them available and you may complete them in our lobby immediately prior to your first session.

Couples Counseling Form:

If you are seeking couples counseling, you may complete the following form (in addition to the above forms) prior to your session. Please bring the completed forms to your first appointment, or we have them available and you may complete them in our lobby immediately prior to your first session.

Insurance Billing Form:

If you want to bill your insurance, you may complete this form (in addition to the previous forms) prior to your session. Please bring the completed forms to your first appointment, or we have them available and you may complete them in our lobby immediately prior to your first session.

Payment:

Payment is due at the time of service. We accept cash or check only. We do not accept credit cards.

LPCs do not bill insurance and have found that insurance companies generally do not cover their services. LPCs try to keep their rates affordable and in special cases will allow sliding fees. They will also provide a receipt upon request for Flex accounts or private insurance billing. Out of Pocket payment allows the most confidential and flexible treatment planning and delivery of services.

LPCCs will bill insurance at your request if they are a provider with your insurance company. You will be responsible for copayment at time of service. If services are covered by insurance, a diagnosis and considerable paperwork are usually required and remain part of your permanent medical record.
